True Strike is an item perk made at Item Workbench. Its compatible equipment is listed above.
What it changes
On supported attacks against monsters, an activation raises the starting damage-roll minimum to a fraction of the attack's calculated maximum. Most supported weapons also bypass the normal defence roll; tridents and the warped sceptre keep their normal accuracy check. Ordinary single-hit attacks keep their starting maximum, but a dark bow's second arrow can also gain a higher maximum.
The minimum is rounded down. Multi-hit attacks do not automatically get a separate activation roll on every hit.
When it helps
Makes supported attacks more reliable against monsters with high defence and removes low damage rolls when it triggers. It is useful when missed hits matter more than raising your maximum hit.
Worked example
For a supported ordinary melee attack with a calculated maximum of 40 and no other modifiers, level 5 has a 15% trigger chance. A trigger skips the normal defence roll and rolls from 20 to 40 instead of 0 to 40. Encounter immunities, damage caps and later reductions can still change the result.

Limits and interactions
Does not affect attacks against players. There is no Wilderness exclusion or cooldown for supported monster attacks. Ordinary Gridmaster attacks are eligible; Gridmaster Echo and Guardian hits do not run this trigger.
Supported attacks include ordinary melee and ranged attacks, spellbook combat spells, training-staff attacks and several powered staffs. Chinchompas can trigger it for the primary target, but their splash hits do not inherit it. Infernal tecpatl, Gauntlet staff attacks, corrupted javelin and Tonalztics of ralos do not receive True Strike.
Trident of the seas, trident of the swamp and warped sceptre attacks raise the minimum when it activates, but still make their normal accuracy roll.
Dark bows apply the effect to their second arrow using the main attack's maximum; this can also raise that arrow's starting maximum. Magma blowpipe applies it to its first dart. Scythe variants reuse the minimum calculated from the main hit for their smaller hits, so the table is not a separate percentage floor for each component.
Ordinary melee and ranged special attacks generally bypass this effect. Skipping the normal defence roll does not bypass encounter-specific blocking or immunity.
Attach and upgrade
At home, open Item Workbench and choose Perks Mode. Select the equipment from your inventory, then choose True Strike. The first application rolls a level from 1 to 5. Applying it again can raise its level; an equal or lower roll keeps the current level and does not refund materials used. Each attempt normally spends the recipe again; eligible donation ranks can preserve individual recipe entries. This does not change the level-roll chances. Level 5 cannot be upgraded further.

Stored and carried materials combine; storage is used first. The return column is for Remove Perk in Perks Mode. It refunds part of one base recipe to Workbench Storage, even after several upgrades. Disintegration destroys the equipment and its attachments.
