Dom is the pet reward from Doom of Mokhaiotl. It is rolled separately from unique equipment and normal loot. The base chance per individual delve kill is 1/1,000 at delve 6, 1/750 at 7, 1/500 at 8 and 1/250 at 9+. Ordinary drop-rate bonuses affect this Doom reward roll.
Dom remains part of the unclaimed run loot until you claim it. Descending risks it with the other rewards; dying before claiming loses it. Claiming records its collection-log and pet-item ownership credit.
